Site wide added external tool not showing up in courses

Using Moodle 2.7, I have added an external tool at site level, but no matter what I do I can't get it to be available when adding at course level.

If I add the LTI at course level, it works in that course, but goes into the Rejected tab at site level and even if I then accept it, it's still not available in other courses.

Any suggestions?
