I've seen that code before and it appears to be a relic of a long since unused workflow, in which the (LTI 1.1) tool would return to the consumer's return url (mod/lti/return.php) with an 'unsigned' param, at which point the consumer site presents a link to the instructor. Clicking the link creates a pending tool and asks them to follow up with the site admin. It was added when the module was, back in 2011.
This really needs to be deprecated and removed, since LTI 1.1 is deprecated, and our 1.3 implementation requires that tool setup is handled by the site administrator anyway. We don't test this legacy tool request feature, nor have we tested this in a long time. Essentially, requesting a tool is an out of band process now and Moodle doesn't provide a feature for this. I believe there are also some legacy site admin views to clean up as part of this deprecation, when we get to it. I'm hoping we can soon. In the meantime, I'd suggest just ignoring this feature entirely as it's for a legacy version of LTI and is also very likely not working.
