I'm don't have any technical background myself but as far I as understood:
LTI uses the server name to 'compose' the right URL.

Our hosting partner used '_' as the default server name (because they define the actual server name at another level). This did not 'cause any issues so far, but this did cause an error when we wanted to set up LTI for the first time.

You can check this yourself on the .../admin/phpinfo.php page.
in our situation it said: $_SERVER['SERVER_NAME'] _
We have now changed the '_' to the actual domain (e.g.client.domain.be)
By changing this, the problem was solved as the LTI could now use the domain as part of the URL.
