Publish to LTI URL link in course refused to connect

When Moodle is the LTI consumer, any URL inside a Moodle course, for example, will throw an error "connection refused". 

Presumably because it is trying to open within the embedded frame.

Other LMS (Canvas, etc) can handle this and open the link in another tab.
